Recent years have seen a growing tendency for social scientists to collect biological specimens such as blood, urine, and saliva as part of large-scale household surveys. By combining biological and social data, scientists are opening up new fields of inquiry and are able for the first time to address many new questions and connections. But including biospecimens in social surveys also adds a great deal of complexity and cost to the investigator's task. Along with the usual concerns about informed consent, privacy issues, and the best ways to collect, store, and share data, researchers now face a variety of issues that are much less familiar or that appear in a new light. The 2010 Gulf of Mexico oil spill is unique in its magnitude, its duration, and in the complexity of its assessment. From the origin of the leak, to the amount of oil released into the environment, to the spill's duration, the Gulf oil spill poses unique challenges to human health. The risks associated with extensive, prolonged use of dispersants, with oil fumes, and with particulate matter from controlled burns are also uncertain. There have been concerns about the extent to which hazards, such as physical and chemical exposures and social and economic disruptions, will impact the overall health of people who live and work near the area of the oil spill.
Although studies of previous oil spills provide some basis for identifying and mitigating the human health effects of these exposures, the existing data are insufficient to fully understand and predict the overall impact of hazards from the Deepwater Horizon oil spill on the health of workers, volunteers, residents, visitors, and special populations. Assessing the Effects of the Gulf of Mexico Oil Spill on Human Health identifies populations at increased risks for adverse health effects and explores effective communication strategies to convey health information to these at-risk populations. The book also discusses the need for appropriate surveillance systems to monitor the spill's potential short- and long-term health effects on affected communities and individuals.
Assessing the Effects of the Gulf of Mexico Oil Spill on Human Health is a useful resource that can help policy makers, public health officials, academics, community advocates, scientists, and members of the public collaborate to create a monitoring and surveillance system that results in "actionable" information and that identifies emerging health risks in specific populations.

As the U.S. health care system continues to evolve, the role of nurses also needs to evolve. Nurses must strike a delicate balance among advancing science, translating and applying research, and caring for individuals and families across all settings. Preparing nurses to achieve this balance is a significant challenge. The education system should ensure that nurses have the intellectual capacity, human responsiveness, flexibility, and leadership skills to provide care and promote health whenever and wherever needed. Education leaders and faculty need to prepare nurses with the competencies they need now and in the future. In some coalbeds, naturally occurring water pressure holds methane--the main component of natural gas--fixed to coal surfaces and within the coal. In a coalbed methane (CBM) well, pumping water from the coalbeds lowers this pressure, facilitating the release of methane from the coal for extraction and use as an energy source. Water pumped from coalbeds during this process--CBM 'produced water'--is managed through some combination of treatment, disposal, storage, or use, subject to compliance with federal and state regulations. Select Agents are defined in regulations through a list of names of particularly dangerous known bacteria, viruses, toxins, and fungi. However, natural variation and intentional genetic modification blur the boundaries of any discrete Select Agent list based on names. Access to technologies that can generate or 'synthesize' any DNA sequence is expanding, making it easier and less expensive for researchers, industry scientists, and amateur users to create organisms without needing to obtain samples of existing stocks or cultures. This has led to growing concerns that these DNA synthesis technologies might be used to synthesize Select Agents, modify such agents by introducing small changes to the genetic sequence, or create entirely new pathogens. Amid these concerns, the National Institutes of Health requested that the Research Council investigate the science and technology needed to replace the current Select Agent list with an oversight system that predicts if a DNA sequence could be used to produce an organism that should be regulated as a Select Agent. The Department of Defense (DOD) supports basic research to advance fundamental knowledge in fields important to national defense. Over the past six years, however, several groups have raised concern about whether the nature of DOD-funded basic research is changing. The concerns include these: Funds are being spent for research that does not fall under DOD’s definition of basic research; reporting requirements have become cumbersome and onerous; and basic research is handled differently by the three services. Soldiers deployed during the 1991 Persian Gulf War were exposed to high concentrations of particulate matter (PM) and other airborne pollutants. Their exposures were largely the result of daily windblown dust, dust storms, and smoke from oil fires. On returning from deployment, many veterans complained of persistent respiratory symptoms. With the renewed activity in the Middle East over the last few years, deployed military personnel are again exposed to dust storms and daily windblown dust in addition to other types of PM, such as diesel exhaust and particles from open-pit burning. On the basis of the high concentrations observed and concerns about the potential health effects, DOD designed and implemented a study to characterize and quantify the PM in the ambient environment at 15 sites in the Middle East. Despite spending more time and money in developing novel therapeutics, the success rate for new pharmacologic treatments has been poor. Although the research and development expenditures have grown 13 percent each year since 1970 (a 50-fold increase), the number of new drugs approved annually is no greater now than it was 50 years ago. Over the past decade, skyrocketing costs and the complexity of the scientific knowledge upon which to develop new agents have provided incentives for alternative approaches to drug development, if we are to continue to improve clinical care and reduce mortality. Since genetically engineered (GE) crops were introduced in 1996, their use in the United States has grown rapidly, accounting for 80-90 percent of soybean, corn, and cotton acreage in 2009. To date, crops with traits that provide resistance to some herbicides and to specific insect pests have benefited adopting farmers by reducing crop losses to insect damage, by increasing flexibility in time management, and by facilitating the use of more environmentally friendly pesticides and tillage practices. However, excessive reliance on a single technology combined with a lack of diverse farming practices could undermine the economic and environmental gains from these GE crops. All six species of sea turtles found in U.S. waters are listed as endangered or threatened, but the exact population sizes of these species are unknown due to a lack of key information regarding birth and survival rates. California's Bay-Delta estuary is a biologically diverse estuarine ecosystem that plays a central role in the distribution of California's water from the state's wetter northern regions to its southern, arid, and populous cities and agricultural areas. Recently, the Fish and Wildlife Service and the National Marine Fisheries Service required changes (reasonable and prudent alternatives, or RPAs) in water operations and related actions to avoid jeopardizing the continued existence and potential for recovery of threatened species of fish. The public-private partnership to develop vehicles that require less petroleum-based fuel and emit fewer greenhouse gases should continue to include fuel cells and other hydrogen technologies in its research and development portfolio. The FreedomCAR (Cooperative Automotive Research) and Fuel Partnership is a research collaboration among the U.S. Department of Energy, the United States Council for Automotive Research - whose members are the Detroit automakers--five major energy companies, and two electric utility companies. The partnership seeks to advance the technologies essential for components and infrastructure for a full range of affordable, clean, energy efficient cars and light trucks. Until recently, the program primarily focused on developing technologies that would allow U.S. automakers to make production and marketing decisions by 2015 on hydrogen fuel cell-powered vehicles. These vehicles have the potential to be much more energy-efficient than conventional gasoline-powered vehicles, produce no harmful tailpipe emissions, and significantly reduce petroleum use. Natural gas, composed mostly of methane, is the cleanest of all the fossil fuels, emitting 25-50% less carbon dioxide than either oil or coal for each unit of energy produced. In recent years, natural gas supplied approximately 20-25% of all energy consumed in the United States. Methane hydrate is a potentially enormous and as yet untapped source of methane. The Department of Energy's Methane Hydrate Research and Development Program has been tasked since 2000 to implement and coordinate a national methane hydrate research effort to stimulate the development of knowledge and technology necessary for commercial production of methane from methane hydrate in a safe and environmentally responsible way. In 1950, men and women in the United States had a combined life expectancy of 68.9 years, the 12th highest life expectancy at birth in the world. Today, life expectancy is up to 79.2 years, yet the country is now only 28th highest, behind the United Kingdom, Korea, Canada, and France, among others. The United States does have higher rates of infant mortality and violent deaths than in other developed countries, but these factors do not fully account for the country's relatively poor ranking in life expectancy. International Differences in Mortality at Older Ages: Dimensions and Sources examines patterns in international differences in life expectancy above age 50 and assesses the evidence and arguments that have been advanced to explain the poor position of the United States relative to other countries. Helium has long been the subject of public policy deliberation and management, largely because of its many strategic uses and its unusual source-it is a derived product of natural gas and its market has several anomalous characteristics. Shortly after sources of helium were discovered at the beginning of the last century, the U.S. government recognized helium's potential importance to the nation's interests and placed its production and availability under strict governmental control. In the 1960s, helium's strategic value in cold war efforts was reflected in policies that resulted in the accumulation of a large reserve of helium owned by the federal government. The latest manifestation of public policy is expressed in the Helium Privatization Act of 1996 (1996 12 Act), which directs that substantially all of the helium accumulated as a result of those earlier policies be sold off by 2015 at prices sufficient to repay the federal government for its outlays associated with the helium program. Cardiovascular disease (CVD), once thought to be confined primarily to industrialized nations, has emerged as a major health threat in developing countries. Cardiovascular disease now accounts for nearly 30 percent of deaths in low and middle income countries each year, and is accompanied by significant economic repercussions. Yet most governments, global health institutions, and development agencies have largely overlooked CVD as they have invested in health in developing countries. Recognizing the gap between the compelling evidence of the global CVD burden and the investment needed to prevent and control CVD, the National Heart, Lung, and Blood Institute (NHLBI) turned to the IOM for advice on how to catalyze change. In this report, the IOM recommends that the NHLBI, development agencies, nongovernmental organizations, and governments work toward two essential goals: creating environments that promote heart healthy lifestyle choices and help reduce the risk of chronic diseases, and building public health infrastructure and health systems with the capacity to implement programs that will effectively detect and reduce risk and manage CVD. The world's nations are moving toward agreements that will bind us together in an effort to limit future greenhouse gas emissions. With such agreements will come the need for all nations to make accurate estimates of greenhouse gas emissions and to monitor changes over time. In this context, the present book focuses on the greenhouse gases that result from human activities, have long lifetimes in the atmosphere and thus will change global climate for decades to millennia or more, and are currently included in international agreements. The book devotes considerably more space to CO2 than to the other gases because CO2 is the largest single contributor to global climate change and is thus the focus of many mitigation efforts. The success of any plan for improving educational outcomes depends on the teachers who carry it out and thus on the abilities of those attracted to the field and their preparation. Yet there are many questions about how teachers are being prepared and how they ought to be prepared. Yet, teacher preparation is often treated as an afterthought in discussions of improving the public education system. Preparing Teachers addresses the issue of teacher preparation with specific attention to reading, mathematics, and science. The book evaluates the characteristics of the candidates who enter teacher preparation programs, the sorts of instruction and experiences teacher candidates receive in preparation programs, and the extent that the required instruction and experiences are consistent with converging scientific evidence. Preparing Teachers also identifies a need for a data collection model to provide valid and reliable information about the content knowledge, pedagogical competence, and effectiveness of graduates from the various kinds of teacher preparation programs. For the United States, the 1991 Persian Gulf War was a brief and successful military operation with few injuries and deaths. However, soon after returning from duty, a large number of veterans began reporting health problems they believed were associated with their service in the Gulf. At the request of Congress, the IOM is conducting an ongoing review of the evidence to determine veterans' long-term health problems and what might be causing those problems. The fourth volume in the series, released in 2006, summarizes the long-term health problems seen in Gulf War veterans. Consumption of goods and services represents a growing share of global economic activity. In the United States, consumption accounts for more than two-thirds of gross domestic product. This trend of increasing consumption has brought with it negative consequences for the environment and human well-being. Global demand for energy, food, and all manner of goods is on the rise, putting strains on the natural and human capital required to produce them. Extractive industries and production processes are prominent causes of species endangerment. Modern economies are underpinned by substantial energy consumption, a primary contributor to the current climate crisis. Expanding international trade has led to many economic opportunities, but has also contributed to unfair labor practices and wealth disparities. While certain processes have improved or become more efficient, and certain practices have been outlawed or amended, the sheer scale of global consumption and its attendant impacts continue to be major challenges we face in the transition to sustainability. Third-party certification systems have emerged over the last 15 years as a tool with some promise. There has been anecdotal evidence of success, but to date the overall impact of certified goods and services has been small. Moreover, definitions of sustainable vary across sectors and markets, and rigorous assessments of these programs have been few and far between. In order to take a step in learning from this field of practice, the National Academies' Science and Technology for Sustainability Program held a workshop to illuminate the decision making process of those who purchase and produce certified goods and services. There is a national need to educate the public about the ocean, coastal resources, atmosphere and climate. The National Oceanic and Atmospheric Administration (NOAA), the agency responsible for understanding and predicting changes in the Earth's environment and conserving and managing coastal and marine resources to meet the nation's economic, social and environmental needs, has a broad mandate to engage and coordinate education initiatives on these topics. Since its creation in 1970, the NOAA has supported a variety of education projects that cover a range of topics related to the agency's scientific and stewardship mission. NOAA uses formal and informal learning environments to enhance understanding of science, technology, engineering and mathematics (STEM) and to advance environmental education. The work of this agency overlaps and compliments the missions of other federal agencies, institutions of higher education, private and nonprofit organizations. Coordination among these agencies and organizations has been challenging. Limited education resources and the inherently global nature of NOAA's mission make strategic partnerships critical in order for the agency to accomplish its goals. Additionally, clear education goals, planning, and strategic use of resources are critical aspects for effective partnerships. NOAA's Education Program: Review and Critique provides a summary of the national education context for NOAA's role in education which is twofold: first is to advance the environmental literacy of the nation, and second is to promote a diverse workforce in ocean, coastal, Great Lakes, atmospheric and climate sciences. The United States spends approximately four million dollars each year searching for near-Earth objects (NEOs). The objective is to detect those that may collide with Earth. The majority of this funding supports the operation of several observatories that scan the sky searching for NEOs. This, however, is insufficient in detecting the majority of NEOs that may present a tangible threat to humanity. A significantly smaller amount of funding supports ways to protect the Earth from such a potential collision or "mitigation." In 2005, a Congressional mandate called for NASA to detect 90 percent of NEOs with diameters of 140 meters of greater by 2020. Defending Planet Earth: Near-Earth Object Surveys and Hazard Mitigation Strategies identifies the need for detection of objects as small as 30 to 50 meters as these can be highly destructive. The book explores four main types of mitigation including civil defense, "slow push" or "pull" methods, kinetic impactors and nuclear explosions. A major goal of the US Department of Defense (DOD) Transformational Medical Technologies Initiative (TMTI) is to develop countermeasures that will protect military personnel against bioweapons, including specific infectious-disease agents and toxins. An explicit TMTI objective is to respond quickly to such threats by producing an appropriate amount of an effective countermeasure--currently defined as enough material to treat or vaccinate 3 million personnel--within 12 months of identification of a specific threat. DOD officials call for TMTI programs to be up and running by 2014. The National Academies hosted a workshop which brought together scientists from academe, government, and the biotechnology industry to identify and discuss challenges and ideas related to the TMTI's vision of developing countermeasures within a few months after an agent is identified. The workshop focused on manufacturing processes and specifically on the development of "manufacturing platforms"--repeatable components of manufacturing that reduce both development time and risk. One of the most critical issues facing the United States today is the proper management of our water resources. Water availability and quality are changing due to increasing population, urbanization, and land use and climate change, and shortages in water supply have been increasing in frequency in many parts of the country. The National Science Foundation (NSF) has entertained the Water and Environmental Research Systems (WATERS) Network as one possible initiative whereby NSF could provide the advances in the basic science needed to respond effectively to the challenge of managing water resources. The WATERS Network, a joint initiative of the Engineering, the Geosciences, and the Social, Behavioral and Economic Sciences directorates at NSF, is envisioned as an integrated national network of observatories and experimental facilities supporting research, outreach, and education on large-scale, water-related environmental problems. In 2007, the National Research Council envisioned a new paradigm in which biologically important perturbations in key toxicity pathways would be evaluated with new methods in molecular biology, bioinformatics, computational toxicology, and a comprehensive array of in vitro tests based primarily on human biology. Although some considered the vision too optimistic with respect to the promise of the new science, no one can deny that a revolution in toxicity testing is under way. New approaches are being developed, and data are being generated. As a result, the U.S. Environmental Protection Agency (EPA) expects a large influx of data that will need to be evaluated. EPA also is faced with tens of thousands of chemicals on which toxicity information is incomplete and emerging chemicals and substances that will need risk assessment and possible regulation. Therefore, the agency asked the National Research Council to convene a symposium to stimulate discussion on the application of the new approaches and data in risk assessment. In March and early April 2009, a new, swine-origin 2009-H1N1 influenza A virus emerged in Mexico and the United States. During the first few weeks of surveillance, the virus spread by human-to-human transmission worldwide to over 30 countries. On June 11, 2009, the World Health Organization (WHO) raised the worldwide pandemic alert level to Phase 6 in response to the ongoing global spread of the novel influenza A (H1N1) virus. By October 30, 2009, the H1N1 influenza A had spread to 191 countries and resulted in 5,700 fatalities. A national emergency was declared in the United States and the swine flu joined SARS and the avian flu as pandemics of the 21st century. Vaccination is currently available, but in limited supply, and with a 60 percent effectiveness rate against the virus. The story of how this new influenza virus spread out of Mexico to other parts of North America and then on to Europe, the Far East, and now Australia and the Pacific Rim countries has its origins in the global interconnectedness of travel, trade, and tourism. Given the rapid spread of the virus, the international scientific, public health, security, and policy communities had to mobilize quickly to characterize this unique virus and address its potential effects. The World Health Organization and Centers for Disease Control have played critical roles in the surveillance, detection and responses to the H1N1 virus. The Domestic and International Impacts of the 2009-H1N1 Influenza A Pandemic: Global Challenges, Global Solutions aimed to examine the evolutionary origins of the H1N1 virus and evaluate its potential public health and socioeconomic consequences, while monitoring and mitigating the impact of a fast-moving pandemic. Recent outbreaks of illnesses traced to contaminated sprouts and lettuce illustrate the holes that exist in the system for monitoring problems and preventing foodborne diseases. Although it is not solely responsible for ensuring the safety of the nation's food supply, the U.S. Food and Drug Administration (FDA) oversees monitoring and intervention for 80 percent of the food supply. The U.S. Food and Drug Administration's abilities to discover potential threats to food safety and prevent outbreaks of foodborne illness are hampered by impediments to efficient use of its limited resources and a piecemeal approach to gathering and using information on risks. Enhancing Food Safety: The Role of the Food and Drug Administration, a new book from the Institute of Medicine and the National Research Council, responds to a congressional request for recommendations on how to close gaps in FDA's food safety systems. Enhancing Food Safety begins with a brief review of the Food Protection Plan (FPP), FDA's food safety philosophy developed in 2007. The lack of sufficient detail and specific strategies in the FPP renders it ineffectual. The book stresses the need for FPP to evolve and be supported by the type of strategic planning described in these pages. It also explores the development and implementation of a stronger, more effective food safety system built on a risk-based approach to food safety management. Various combinations of commercially available technologies could greatly reduce fuel consumption in passenger cars, sport-utility vehicles, minivans, and other light-duty vehicles without compromising vehicle performance or safety. Assessment of Technologies for Improving Light Duty Vehicle Fuel Economy estimates the potential fuel savings and costs to consumers of available technology combinations for three types of engines: spark-ignition gasoline, compression-ignition diesel, and hybrid. According to its estimates, adopting the full combination of improved technologies in medium and large cars and pickup trucks with spark-ignition engines could reduce fuel consumption by 29 percent at an additional cost of $2,200 to the consumer. Replacing spark-ignition engines with diesel engines and components would yield fuel savings of about 37 percent at an added cost of approximately $5,900 per vehicle, and replacing spark-ignition engines with hybrid engines and components would reduce fuel consumption by 43 percent at an increase of $6,000 per vehicle. The book focuses on fuel consumption--the amount of fuel consumed in a given driving distance--because energy savings are directly related to the amount of fuel used. In contrast, fuel economy measures how far a vehicle will travel with a gallon of fuel. Because fuel consumption data indicate money saved on fuel purchases and reductions in carbon dioxide emissions, the book finds that vehicle stickers should provide consumers with fuel consumption data in addition to fuel economy information. The Commercial Buildings Energy Consumption Survey (CBECS) is a survey of commercial buildings in the United States, mandated by Congress to provide comprehensive information about energy use in commercial buildings. In addition to energy consumption and expenditure data, the survey collects information about building characteristics, such as energy source, physical structure, equipment used, and activities performed, which provides researchers with detailed information about commercial sector energy use and how it relates to building characteristics. The CBECS is the only national source of these data, and is used for energy forecasting, program development, and policy development. At the request of the Energy Information Administration, the National Research Council is conducting a comprehensive 30-month study of the CBECS and the corresponding study of Residential Energy Consumption Survey (RECS). Because plans for the upcoming 2011 round of CBECS must be finalized in the near future, the panel was charged to comment as soon as possible on design and data collection options that would enable the upcoming round of this survey to better support U.S. Department of Energy program information needs, reduce respondent burden, and increase the quality and timeliness of the data. This letter responds to that request, and is limited in scope to discussing issues that the panel believes are realistic to consider in the timeframe leading up to the 2011 data collection. During medical emergencies, hospital staff and emergency medical services (EMS) providers, can face barriers in delivering the fastest and best possible care. Overcrowded emergency rooms cannot care for patients as quickly as necessary, and some may divert ambulances and turn away new patients outright. In many states, ambulance staff lacks the means to determine which hospitals can provide the best care to a patient. Given this absence of knowledge, they bring patients to the closest hospital. In addition, because emergency service providers from different companies compete with each other for patients, and emergency care legislation varies from state to state, it is difficult to establish the necessary local, interstate, and national communication and collaboration to create a more efficient system. In 2006, the IOM recommended that the federal government implement a regionalized emergency care system to improve cooperation and overcome these challenges. In a regionalized system, local hospitals and EMS providers would coordinate their efforts so that patients would be brought to hospitals based on the hospitals' capacity and expertise to best meet patients' needs. In September 2009, three years after making these recommendations, the IOM held a workshop sponsored by the federal Emergency Care Coordination Center to assess the nation's progress toward regionalizing emergency care. The workshop brought together policymakers and stakeholders, including nurses, EMS personnel, hospital administrators, and others involved in emergency care. During public health emergencies such as pandemic influenza outbreaks or terrorist attacks, effective vaccines, drugs, diagnostics, and other medical countermeasures are essential to protecting national security and the public's well-being. The Public Health Emergency Medical Countermeasures Enterprise (PHEMCE--a partnership among federal, state, and local governments; industry; and academia--is at the forefront of the effort to develop and manufacture these countermeasures. However, despite the PHEMCE's many successes, there are still serious challenges to overcome. Government-funded medical research is not always focused on countermeasures for the most serious potential threats, and it is difficult to engage pharmaceutical and biotechnology companies to develop and manufacture medical countermeasures that have a limited commercial market. At the request of the Secretary of the U.S. Department of Health and Human Services and the Assistant Secretary for Preparedness and Response, the IOM held a workshop February 22-24, 2010, to address challenges facing the PHEMCE. In the military, information technology (IT) has enabled profound advances in weapons systems and the management and operation of the defense enterprise. A significant portion of the Department of Defense (DOD) budget is spent on capabilities acquired as commercial IT commodities, developmental IT systems that support a broad range of warfighting and functional applications, and IT components embedded in weapons systems. The ability of the DOD and its industrial partners to harness and apply IT for warfighting, command and control and communications, logistics, and transportation has contributed enormously to fielding the world's best defense force. However, despite the DOD's decades of success in leveraging IT across the defense enterprise, the acquisition of IT systems continues to be burdened with serious problems. To address these issues, the National Research Council assembled a group of IT systems acquisition and T&E experts, commercial software developers, software engineers, computer scientists and other academic researchers. The group evaluated applicable legislative requirements, examined the processes and capabilities of the commercial IT sector, analyzed DOD's concepts for systems engineering and testing in virtual environments, and examined the DOD acquisition environment. The present volume summarizes this analysis and also includes recommendations on how to improve the acquisition, systems engineering, and T&E processes to achieve the DOD's network-centric goals. Despite the many benefits of energy, most of which are reflected in energy market prices, the production, distribution, and use of energy causes negative effects. Many of these negative effects are not reflected in energy market prices. When market failures like this occur, there may be a case for government interventions in the form of regulations, taxes, fees, tradable permits, or other instruments that will motivate recognition of these external or hidden costs.The Hidden Costs of Energy defines and evaluates key external costs and benefits that are associated with the production, distribution, and use of energy, but are not reflected in market prices. The damage estimates presented are substantial and reflect damages from air pollution associated with electricity generation, motor vehicle transportation, and heat generation. The book also considers other effects not quantified in dollar amounts, such as damages from climate change, effects of some air pollutants such as mercury, and risks to national security.While not a comprehensive guide to policy, this analysis indicates that major initiatives to further reduce other emissions, improve energy efficiency, or shift to a cleaner electricity generating mix could substantially reduce the damages of external effects. A first step in minimizing the adverse consequences of new energy technologies is to better understand these external effects and damages. The Hidden Costs of Energy will therefore be a vital informational tool for government policy makers, scientists, and economists in even the earliest stages of research and development on energy technologies. The National School Lunch Program and the School Breakfast Program, administered by the Food and Nutrition Service of the U.S. Department of Agriculture (USDA), are key components of the nation's food security safety net, providing free or low-cost meals to millions of school-age children each day. Under the most commonly adopted provisions, USDA reimburses districts for meals served on the basis of data collected in a "base year," during which applications are taken. After 3 or 4 years, applications must be taken again to establish new base-year data, unless the district provides evidence that local conditions have not changed. A special provision that does not require applications to be taken every few years would reduce burden, be more attractive to school districts, and potentially increase student participation by expanding access to free meals. To support the development of such a provision, the Food and Nutrition Service asked the National Academies to study the technical and operational issues that arise in using data from the American Community Survey (ACS)--a new continuous survey replacing the long-form survey of the decennial census--to obtain estimates of students who are eligible for free and reduced-price meals for schools and school districts. Such estimates would be used to develop "claiming percentages" that, if sufficiently accurate, would determine federal reimbursements to districts for the schools that provide free meals to all students under a new special provision that eliminates the base-year requirements of current provisions. Reducing vulnerabilities to impacts of climate change that the nation cannot, or does not, avoid is a highly desirable strategy to manage and minimize the risks, says the book, Adapting to the Impacts of Climate Change. Some impacts--such as rising sea levels, disappearing sea ice, and the frequency and intensity of some extreme weather events like heavy precipitation and heat waves--are already being observed across the country. The book notes that policymakers need to anticipate a range of possible climate conditions and that uncertainty about the exact timing and magnitude of impacts is not a reason to wait to act. In fact, it says boosting U.S. adaptive capacity now can be viewed as "an insurance policy against an uncertain future," while inaction could increase risks, especially if the rate of climate change is particularly large. Over the past 5 years or more, there has been a steady and significant decrease in NASA's laboratory capabilities, including equipment, maintenance, and facility upgrades. This adversely affects the support of NASA's scientists, who rely on these capabilities, as well as NASA's ability to make the basic scientific and technical contributions that others depend on for programs of national importance. The fundamental research community at NASA has been severely impacted by the budget reductions that are responsible for this decrease in laboratory capabilities, and as a result NASA's ability to support even NASA's future goals is in serious jeopardy. The U.S. Army Medical Research Institute of Infectious Diseases in Frederick, Maryland, is designed to handle pathogens that cause serious or potentially lethal diseases, which require the research performed on them be contained to specialized laboratories. In 2007 a decision was made to expand those facilities causing concern among area residents that public health and safety risks, and strategies to mitigate those concerns were not adequately considered in the decision to go forward with the expansion. In Evaluation of the Health and Safety Risks of the New USAMRIID High Containment Facilities at Fort Detrick, Maryland a group of experts in areas including biosafety, infectious diseases, industrial hygiene, environmental engineering, risk assessment and epidemiology, explored whether measures were being taken to ensure prevention and mitigation of risk to the health and safety of workers and the public. They also assessed whether the procedures and regulations employed meet accepted standards of the Centers for Disease Control and Prevention and the National Institutes of Health. Evaluation of the Health and Safety Risks of the New USAMRIID High Containment Facilities at Fort Detrick, Maryland evaluates the health and safety aspects of the environmental impact statement developed to support the construction of the new laboratories and explores the institute's operating requirements, medical and emergency management response plans and communication and cooperation with the public. Information about the characteristics of jobs and the individuals who fill them is valuable for career guidance, reemployment counseling, workforce development, human resource management, and other purposes. To meet these needs, the U.S. Department of Labor (DOL) in 1998 launched the Occupational Information Network (O*NET), which consists of a content model--a framework for organizing occupational data--and an electronic database. The O*NET content model includes hundreds of descriptors of work and workers organized into domains, such as skills, knowledge, and work activities. Data are collected using a classification system that organizes job titles into 1,102 occupations. The National Center for O*NET Development (the O*NET Center) continually collects data related to these occupations. In 2008, DOL requested the National Academies to review O*NET and consider its future directions. In response, the present volume inventories and evaluates the uses of O*NET; explores the linkage of O*NET with the Standard Occupational Classification System and other data sets; and identifies ways to improve O*NET, particularly in the areas of cost-effectiveness, efficiency, and currency. The United States has the highest per capita spending on health care of any industrialized nation. Yet despite the unprecedented levels of spending, harmful medical errors abound, uncoordinated care continues to frustrate patients and providers, and U.S. healthcare costs continue to increase. The growing ranks of the uninsured, an aging population with a higher prevalence of chronic diseases, and many patients with multiple conditions together constitute more complicating factors in the trend to higher costs of care. A variety of strategies are beginning to be employed throughout the health system to address the central issue of value, with the goal of improving the net ratio of benefits obtained per dollar spent on health care. However, despite the obvious need, no single agreed-upon measure of value or comprehensive, coordinated systemwide approach to assess and improve the value of health care exists. Without this definition and approach, the path to achieving greater value will be characterized by encumbrance rather than progress. To address the issues central to defining, measuring, and improving value in health care, the Institute of Medicine convened a workshop to assemble prominent authorities on healthcare value and leaders of the patient, payer, provider, employer, manufacturer, government, health policy, economics, technology assessment, informatics, health services research, and health professions communities. Plug-in hybrid electric vehicles (PHEVs) promise to contribute to both goals by allowing some miles to be driven on electricity drawn from the grid, with an internal combustion engine that kicks in when the batteries are discharged. However, while battery technology has made great strides in recent years, batteries are still very expensive. Transitions to Alternative Transportation Technologies--Plug-in Hybrid Electric Vehicles builds on a 2008 National Research Council report on hydrogen fuel cell vehicles. At the request of the U.S. Environmental Protection Agency (EPA), the present letter report evaluates the effectiveness of EPA's Title 42 program. Title 42 authority was granted to EPA for 5 years, from 2006 to 2011. As that period draws to a close, it was thought that a review of the current program would be appropriate. This letter report first provides some background information on the origin of EPA's Title 42 program and then more detailed information on the committee's task and its approach to the task, comments on implementation of the Title 42 program, and suggestions for strengthening the program. The report concludes with the committee's overall findings and recommendations. Although the Title 42 program at EPA is still evolving, the committee found that the agency has implemented the program appropriately. Most important, the Title 42 appointees have already had a favorable effect on EPA's scientific research even after such a short time since implementation of the program. This report is the first component of a study to assess the implications of climate change for the U.S. Naval Services. Specifically, this report highlights issues that could have potential near-term impacts, impose a need for near-term awareness, or require near-term planning to ensure that longer-term naval capabilities are protected. In 2003, the Boston University Medical Center (BUMC) was awarded a $128 million grant from the National Institutes of Health to build one of two high- and maximum- containment laboratory facilities for research on biological pathogens. The National Emerging Infectious Disease Laboratories (NEIDL) are meant to support the National Institute of Allergy and Infectious Diseases and biodefense research agenda, conducting research to develop new approaches to treating, preventing, and diagnosing a variety of viral diseases.
